Sports Forms and Handbook
In the event of a concussion, Chelsea's concussion form must be filled out and turned in to the school.
The VHSL Physical Form must be completed in order to participate in athletics. Pages 1-3 must be submitted to the Athletic Director. Pages 4 and 5 remain with the attending physician for their records.
