*This event is exclusively for 9th, 10th, 11th and 12th grade students.

We ask that you carefully read our school's rules and policies, which specifically reference this high school event and will be strictly adhered to.

Finally, any Chelsea student who would like to bring a date from outside of Chelsea Academy MUST fill out and turn in this guest permission form.
