Admissions Forms

The following forms should be downloaded and used in applying to Chelsea Academy:

• Application Form
• Request for Academic Records
• Teacher Recommendation Form

The Grant and Aid Application may be completed online at www.factstuitionaid.com.
 
Step 1: Select Applicant Sign In
Step 2: New Users select Create an Account OR Returning Users enter their existing Username and Password.
 
Alternatively, a paper application may be requested by calling the school office at (540) 635-0622.
